- Step 1: Schedule My First Appointment To schedule your first appointment, call our Appointment Line at 256-666-4195.
- Step 2: Complete New Patient Registration Forms Save time by filling out your New Patient Registration Forms online. Once you’ve booked your appointment, you’ll receive a link to complete the forms electronically. Please follow the prompts to ensure your registration is complete. If online registration is not possible for you, please arrive 30 minutes before your scheduled appointment to register in person. Keep in mind that even if you register online, there may be additional forms to complete.
