If you don’t have insurance or if your insurance coverage is limited, you may be eligible for our Adjustable Payment Plan (APP). This program is designed to make healthcare affordable by offering services on a sliding fee scale.
To be eligible for the program, you need to meet two criteria. First, you must be uninsured or underinsured. Second, your gross family income should be at or below 200% of the Federal Poverty Guidelines. We determine your eligibility by considering the gross income of everyone in your household and the size of your family.

At our Family Health Centers, the cost of your healthcare will depend on what you qualify for. We consider two factors: your family size and your gross family income. For medical visits, the starting cost is $25, but it can increase based on what you qualify for. We also offer a range of other services such as lab tests, chiropractic care, dental services, and more. The cost for these services is determined on a sliding fee scale, ensuring that it is affordable for you and your family. Once we determine your eligibility for the program, we will provide you with all the details about your patient responsibility.
Yes, all services provided at our Family Health Centers are covered under the APP (Adjustable Payment Plan). This means that when you visit us, you can receive the care you need without worrying about unaffordable costs. However, it’s important to note that services outside of our clinics, such as visits to specialists, are not covered under the program.
